There are many general evidences to show that raising the hands for dua'a is a mustahab act and also a sunnah, but there are certain ahadeeth which some scholars have used as corroborating evidence to support raising the hands to make dua'a after fardh salah.
Note : this post does not encourage making dua'a in jamaat (together) after the fardh salah (because there is no evidence that it is a sunnah) but only enlists some narrations showing that raising the hands after fardh salah is okay or in other words allowed.
An important point to be noted is that the Prophet (s.a.w) was not of the habit to raise his hands immediately after the salah, rather he made dhikr (i.e recited the dua's) first and then as per some narrations he raised his hands sometimes to make dua'as. Anwar Shah Kashmeeri says: One should know that ad Du'a al Ma'mool (supplication which is acted upon) in our times after obligatory prayers (by) raising hands (and) upon such guise, (then) there was no persistence on this in the time of Messenger of Allah عليه الصلاة والسلام Yes Many supplications are proven after obligatory prayers (but) without raising hands and without (doing it) in congregation and evidences of it are Mutawatrir.

Also, in addition to the below narrations, some scholars permit raising the hands after salah to make dua'a using the general evidence wherein a person is allowed to supplicate lifting his hands
A HASSAN HADEETH
حَدَّثَنَا سُلَيْمَانُ بْنُ الْحَسَنِ الْعَطَّارُ، قَالَ: حَدَّثَنَا أَبُو كَامِلٍ الْجَحْدَرِيُّ، قَالَ: حَدَّثَنَا الْفُضَيْلُ بْنُ سُلَيْمَانَ، قَالَ: حَدَّثَنَا مُحَمَّدُ بْنُ أَبِي يَحْيَى، قَالَ: رَأَيْتُ عَبْدَ اللهِ بْنَ الزُّبَيْرِ وَرَأَى رَجُلًا رَافِعًا يَدَيْهِ بِدَعَوَاتٍ قَبْلَ أَنْ يَفْرُغَ مِنْ صَلَاتِهِ، فَلَمَّا فَرَغَ مِنْهَا، قَالَ: «إِنَّ رَسُولَ اللهِ صَلَّى اللهُ عَلَيْهِ وَسَلَّمَ لَمْ يَكُنْ يَرْفَعْ يَدَيْهِ حَتَّى يَفْرُغَ مِنْ صَلَاتِهِ»
It is narrated from Abdullah bin az-Zubayr (radiallah anhu) that he saw a man making duaa while raising his hands before the finishing of the prayer. So when he finished his prayer, Abdullah bin az-Zubayr (radiallah anhu) [stopped him from doing that and he] said to him:
"The Prophet (peace be upon him) would not raise his hands until after he finished his prayer"

حَدَّثَنَا أَبِي، ثنا أَبُو مَعْمَرٍ الْمِنْقَرِيُّ، ثنا عَبْدُ الْوَارِثِ، ثنا عَلِيُّ بْنُ زَيْدٍ، عَنْ سَعِيدِ بْنِ الْمُسَيِّبِ، عَنْ أَبِي هُرَيْرَةَ، أَنَّ رَسُولَ اللَّهِ صَلَّى اللهُ عَلَيْهِ وَسَلَّمَ رَفَعَ يَدَهُ بَعْدَ مَا سَلَّمَ وَهُوَ مُسْتَقْبِلٌ الْقِبْلَةَ، فَقَالَ: «اللَّهُمَّ خَلِّصِ الْوَلِيدَ بْنَ الْوَلِيدِ، وَعَيَّاشَ بْنَ أَبِي رَبِيعَةَ، وَسَلَمَةَ بْنَ هِشَامٍ، وَضَعَفَةَ الْمُسْلِمِينَ الَّذِينَ لَا يَسْتَطِيعُونَ حِيلَةً وَلَا يَهْتَدُونَ سَبِيلًا مِنْ أَيْدِي الْكُفَّارِ»
My Father (Imaam Abu Haatim ar-Raazi) narrated to us, Abu Ma'mar al-Minqari narrated to us, Abdul Waarith narrated to us, Ali bin Zayd narrated to us, from Sa'eed bin al-Musayyab, from Abu Hurayrah (radiallah anhu), that the Messenger of Allaah (sallallahu alayhi wasallam) raised his hands after making salaam, and then whilst facing in the direction of the Qiblah, he said: "O Allaah, liberate Al-Waleed bin al-Waleed, Ayyaash bin Abi Rabee'ah, Salamah bin Hishaam, and the weak muslims, who neither have the capability of deception nor being guided on the path, from the hands of the Kuffaar"

This hadeeth is weak because of the weakness of Ali bin Zayd bin Jud'aan.
1- Haafidh al-Haythami writes: “The Jamhoor (Majority) has weakened him"

2- Allaamah Busayri said: “And the Majority (Jamhoor) is upon his being weak”
[Misbaah uz-Zajaajah: 84]
3- Haafidh Ibn al-Iraaqi said: “The Jamhoor has weakened him”

4- Abu al-Hasan Ibraaheem bin Umar al-Buqaa’ee said: “The Jamhoor (of Muhadditheen) have declared him weak”

5- Haafidh an-Nawawi said: “And he is Da’eef according to the Muhadditheen”

Abu Wada’ah (Allah be pleased with him) narrates that the Messenger of Allah (Allah bless him & give him peace) said: “The (optional) prayer of the night should be performed in sets of two Rak’ats, meaning you recite the Tashahhud after every two Rak’ah, express devotion and humility in your supplication, raise your hands and say: “O Allah, forgive me”. Whosoever fails to do so, his prayer will be incomplete.”
Narrated Muttalib:
The Prophet (sallallahu 'alaihi wa sallam) said: Prayer is to be offered in two rak'ahs; and you should recite the tashahhud at the end of two rak'ahs, and express your distress and humility and raise your hands and say praying: O Allah, O Allah. He who does not do so does not offer a perfect prayer.
Abu Dawud was asked about offering prayer at night in two rak'ahs. He said: They may be two if you like and four if you like.

Anas ibn Malik (Allah be pleased with him) narrates that the Messenger of Allah (Allah bless him & give him peace) said: “There is not a servant of Allah who raises his hands after prayer, then says: O Allah, my Lord, Lord of Ibrahim, Lord of Ishaq, Lord of Ya’qub, Lord of Jibril, Lord of Mika’il, Lord of Israfil, I ask You that answer my prayer………….except that Allah takes it upon himself not to reject his (raising) of hands empty.”

Sheikh Saleem Al-Hilaalee said the following: ‘The chain of this narration is extremely weak because of the following defects:
(i) The narrator Khaseef Al-Jazaree, he is Daeef (weak), and did not narrate from Anas bin Malik.
(ii) The narrator Abdul-Aziz bin Abdur-Rahman Al-Qurshi, is Matrook (abandoned), as Imam Ahmed bin Hanbal accused him of lying when narrating.
(iii) The narrator Ishaaq Al-Baalasee is Munkirul-Hadith (narrator of an Unfamiliar Hadith), as mentioned by Ibn Adee.)
FATAAWA
In Fataawa `Ulema Ahl al-hadeeth, Volume 1, Kitaabus-Salah the scholars have said that raising the hands after fardh salah or for du`a is proven from the prophet and likewise from the sahaaba and this `amal is permissible and recommended
CONCLUSION
There are many narrations that relate to the raising of hands whilst supplicating, generally but very few narrations that indicate the sunnah of raising the hands specifically after the salah. The great Jurist, Hadeeth expert and scholar, Imam al-Suyuti (Allah have mercy on him) mentions in his Tadrib al-Rawi that there are approximately 100 narrations with regards to raising the hands whilst supplicating, even though they concern separate incidents (i.e at different situations and authenticity), thus reaching the level of certainty (Tawatur bi al-Ma’na). [Ref: Tadrib al-Rawi, P. 461]
Thus , if one wishes to raise the hands individually only after completing the dhikr, then he may do so.
